A Clinical Study on Factors Influencing Survival Rate in Patients with Long Term Hemodialysis




Abstract
This study was designed to analyze the survival rate and influencing factors on survival in long term hemodialysis patients, and to know the correlation between factors.
We studied 134 patients who were treated in hemodialysis unit of Chung-Ang University Yong San Hospital between June 1985 and March 1989. The study group comprised 89 males and 45 females. The mean age of males was 43.5 years (range 19-78 years) and that of the females 46.6 years (range 21-73 years).
The results are as follows.
1) The blood chemistry data revealed BUN 83 (¡¾6.8) mg/dl, creatinine 14.7(¡¾4.3)mg/dl, BUN/ creatinine ratio 6.1 (¡¾2.4), K 5.8 (¡¾0.8) mEq/1, Ca 8.5 (¡¾1.0) mg/dl, P 6.0 (¡¾1.8) mg/dl and Ca x P 56. 2 (¡¾54.6).
2) Interdialytic weight gain was 0.7 kg/day and systolic blood pressure was 158 mmHg and diastolic blood pressure 94 mmHg.
3) There were significant differences in BUN, creatinine, P level between male group and female group.
4) There was no significant difference in factors except K level between the group with or without complications.
5) There was no significant difference in factors except Ca level between the survival group and the death group.
6) The complication rate of males was 29.2% and higher than that of females. The death rate of the complication group was 51.5% and higher than that of the group without complications.
7) The incidence of hypotension during hemodialysis in the complication group was 25.2% and that of the death group 25.7% and these data were higher than those of each control group.
8) The cause of death were that 23 patients (65.7%) died of cardiovascular complication, 2 patients (5.7%) infection, one patient (2.9%) hepatitis and 9 patients (25.7%) refusal of further treatment due to socio-economic problems.
9) One year patient survival was 80.6%, 2 year survival 64.7% and 3 year survival 52.2%.
